The following files have been shared with us by Bill Deatherage of Deatherage Associates, LLC from Broken Arrow, OK who has collected them from a variety of sources. The files are meant as a basic informational resource only. Many of them would need to be modified to suit your local departmental needs and local city laws and ordinances.
