14th Annual Fallen and Living Firefighters Memorial
Golf Tournament

Thursday, September 16, 2010 at Lincoln Park Golf Course
4001 N.E. Grand Blvd, Oklahoma City

Dress is casual shirt and pants
Registration and Lunch at 11:30 a.m. with Tee-off at 1:00 p.m.
Dinner by Ted’s Cafe Escondido and prizes at the Firefighters Museum after golf.
$65 per person or $260 per team (includes green fees, cart, 2 drink tickets, lunch and dinner)
4 Man scramble with shotgun start

Registration is not complete until payment is received.  If you don’t pay, you don’t play.  Registration form and payment must be sent together
and be received no later than September 8th. 

All proceeds benefit the Oklahoma Fallen and Living Firefighters Memorial.

When registering online, the team captain is considered as player 1. 
The team captain must supply an email and phone number and serve
as the point of contact for the other team members.

Hole Sponsorships are available for $160.
Please include this in your total due if sponsoring a hole.
